We began 2024 with Oystehr serving 29,000 FHIR API calls per day. Yesterday Oystehr served 2,300,000 FHIR API calls, an increase of 7,800%.
Typically, usage peaks between 2pm-4pm ET with around 6,000 calls per minute, or 100 per second.
One year ago, we saw just over 2,000 patient visits per month. Today, we average around 170,000 patient bookings per month.
Ottehr didn’t exist at all at the start of 2024. Today it is the first FHIR-native EHR, and it’s open-source. Ottehr supports both telemed and in-person visits, eRx, practice management, customizable paperwork and intake. In Q1 of this year Ottehr will have lab order and results retrieval, and RCM/claims submission.

Prior to releasing Oystehr, we spent a decade building health tech software. For an experienced team like ours, building a full-scope custom EHR from scratch was at least 200,000 person-hours. In other words, a massive effort that only an exceptionally committed and well capitalized organization like One Medical could afford. But the results were phenomenal when done well. Huge improvements in patient and provider satisfaction coupled with significant long-term cost savings.
With the initial release of Oystehr, we reduced the build cost to around 80,000 person-hours. With the release of Ottehr, we reduced the cost to around 40,000 person-hours. In 2025, with improvements to Oystehr and Ottehr, we will reduce the cost to less than 1000 hours in many cases.
In other words, we will make it possible for moderately sized clinical organizations to have an EHR completely customized to their workflows and use-cases, while achieving long-term cost savings relative to any existing monolithic EHR pricing model.
